Symptom

When authenticating with Domino (6.5.4) LDAP in Crystal Vision Server, the following error messages appear:
"trace message: LDAP: LdapQueryForEntries: *QUERY* result: 1 took 281 ms
trace message: SecLdap Error: an error occurred in LdapGetEntries().
trace message: LDAP: Finished Updating Graph...
trace message: SecLdap Error: an unknown error occurred in LdapGetUsers().
trace message: LDAP: The following groups could not be verified: cn=cp-crystalvision; cn=thornleigh
trace message: SecLdap Error: an unknown error occurred in LdapGetUsers().
LDAP Error: Operations error.
trace message: [UID=0;USID=0;ID=148] Update object in database failed
trace message: Commit failed."
