• Error: "Cannot access the repository. (USR0013)  

[repo_proxy 13] SessionFacade::openSessionLogon with user info has failed(Unable to log on: Could not connect to server IOMRPT01:6400.  Please check that the server name is correct, and that the server is running.(hr=#0x80042a01)" when logon to client tools.

Errors in Event Viewer:

• "Server Intelligence Agent: CMS <SIA_Name>.CentralManagementServer has failed and the Server Intelligence Agent has not been able to connect to the cluster. Please verify your CMS port and database settings are correct."
• "The root server reported an error Initialization Failure. (Reason: Database access error. Reason [Microsoft][SQL Native Client]Invalid cursor state.Database access error. Reason [Microsoft][SQL Native Client]Invalid cursor state. Error committing transaction for AddIndex_Dynamically."
