• The Central Management Server (CMS) and File Repository Servers (FRSs) repeatedly crash and restarts
• Messages similar to the following appear in the CMS trace logs
• OracleDatabase.cpp:93) Oracle error found:  ErrorMessage(ORA-00001: unique constraint (BOAUD.ADS_AUDITEE_2) violated), ErrorCode(1)
• ConfChannel: failed to acquire channel key using id = -x‑
• note:  The first message is specific to Oracle.  For the second and third messages, values x and y are variable and are often repeated for many (potentially thousands) of objects.
• FRS core dumps will show messages like the following, the specific point of interest being that the filename is greater than 256 characters.
