Symptom
When a medical record (MR) is requested by user N (nurse), the "Created By" field in table NDAE is correctly populated with 'N'.
However, when user P (physician) borrows the same medical record, the "Created By" field is updated to 'P', and the "Created On" and "Created At" of the same record get changed as well. This behavior is incorrect.
Instead, only the update-related fields, such as "Changed By", "Changed On", and "Changed At", should be modified to reflect the borrowing action (update fields should be adjusted).
It is essential to have a reliable method to identify the original request creator. Typically, users expect the ERUSR field to store the user who created the entry, and the ERDAT field to reflect the entry creation date.
If ERUSR and ERDAT are updated during subsequent actions, the record becomes non-traceable, which may result in failing data audits.
