2006817 - Request closed even with auto-provisioning failure seen in Audit logs

Request has failed due to provisioning errors, it takes an escape path as configured.
Now if you re-try to approve the request from the escape path, the auto-provisioning fails again, but this time the request is closed. Check in Audit logs. (nothing is changed in the connected plugin system).

In this screen shot, the escape path is triggered and then auto-provisioning fails again, but the escape is duplicated. Hence the symptom is met, the request gets closed, despite errors.
