Symptom
Microsoft has provided information at the following blog stating that out of date ActiveX controls will be blocked in Internet Explorer as of September 9th, 2014:

Included in this list are the following Oracle Java Runtime Environment product versions:
Currently:
- Java 2 Platform, Standard Edition (J2SE) 1.4, everything below (but not including) update 43
- J2SE 5.0, everything below (but not including) update 99
- Java SE 6, everything below (but not including) update 111
- Java SE 7, everything below (but not including) update 99
- Java SE 8, everything below (but not including) update 77..
Starting June 14, 2016
- Java 2 Platform, Standard Edition (J2SE) 1.4, everything below (but not including) update 43
- J2SE 5.0, everything below (but not including) update 99
- Java SE 6, everything below (but not including) update 115
- Java SE 7, everything below (but not including) update 101
- Java SE 8, everything below (but not including) update 91
