• Short term options are: create uniqueness with your user's names by adding the middle initial, or disable the 'Hide user name' feature if that is an option for your organization.

• When your organization's Company Settings are configured to hide the Username field (Provisioning > Company Settings > Hide (user name) in the UI.), the username is not available for use as a unique identifier when mass creating forms.
