2183636 - Manage Templates - Old Template Forms have renamed to New Template forms - Performance Management

When a New template is created by Clone or "Copy from an Old template",  all the forms of the Old template are renamed as New template

You need to make sure that the Old Template name and the New Template name are correct in the "Form template Settings" as per the steps below:

1. Go to Admin Center > Performance Management  > Form Template Settings
2. Select the Template name.
3. If the name is Changed here all the forms you launch will have the name mentioned in the Template Name

If the template name is correct there but you're still getting the New template name then:

1. Go to Old Admin Center
2. Open the Company system and Logo Settings
3. Enable the option "All documents will display their form template name at the time the document was created. This option does not support translations of the document name."
