2186781 - CREATE UNIQUE INDEX terminated because a duplicate key

Symptom

The following error happens during an import phase of migration or upgrade.

Database error 1505 at EXE
CREATE UNIQUE INDEX statement terminated because a duplicate key was found for 
the object name '<sid>.<table_name>' and the index name '<Index_name>.

The error could also happen when trying to create a missing table or index.
