2187693 - "Specify Different Template for Each Form" Cannot be Selected for Some E-Mail Notification Templates

Symptom

In E-mail Notification Template Settings, the checkbox next to "Specify Different Template for Each Form" cannot be enabled for some E-Mail Notification Templates.
