2195690 - Error 515 reported by sp_addlogin after upgrade to SAP ASE 15.7 SP122.

Symptom

After upgrading to SAP ASE 15.7 SP122, the sp_addlogin stored procedure reports the following error:

Msg 515, Level 16, State 3:
Server 'SYBASE', Procedure 'sp_extrapwdchecks', Line 67:
Attempt to insert NULL value into column 'name', table 'master.dbo.pwdhistory';.
Command has been aborted.
Password correctly set.
Account unlocked.New login created.
