2238961 - “Enter Value for Request Type” in Template Based Request

Symptom

When the user tries to create a template based request changing the default request from value “Other” to “Self”, the request type field becomes blank. If the user tries to proceed, the system trows an error “Enter Value for Request Type” and the request cannot be created.
