Symptom
When users send forms to the next step, they receive a confirmation popup screen with a comment box:
You are about to submit the finished form to the Completed folder. Once you submit the form, all the content becomes read-only and can no longer be changed.

Resolution
The confirmation popup cannot be removed, but the comment box can, by following the below steps depending the impacted form template:
- Go to Admin Center > Company Settings > Email Notification Templates Settings
- Depending on the type of impacted form, select the following:
- Performance forms: Document Routing Notification
- 360 Multi-Rater forms: 360 Document Complete Notification
- For applicable templates and languages, remove the token [[DOC_COMMENT]]
- Select Save Changes below the Email Body field
Note: [[DOC_COMMENT]] must be removed from the English_US language template even if English US is not used or enabled in the instance. If English US is not enabled, use the Manage Languages tool to enable English US and remove the token from the Document Routing Notification and/or the 360 Document Complete Notification.
Note: In case the comment box is still appearing after executing the above tasks, then it is advised to remove the respective document comment token from all these email notification types as well that are about the following:
Performance form or 360 form --> route, complete, forward, next step, send.
If the email templates are specified for different templates then the token should be removed for the templates where the comment box is not needed otherwise it should be removed for all templates.
Regarding the languages it should be removed for English US in all cases and for other languages that are in use and desired not to have the comment box.
After each removal the changes should be saved in the right side panel at the bottom of the email template box and the "update settings" should be clicked. Upon evey change is done then saving at the bottom of the page is needed.
Comment boxes now should disappear in all cases upon routing the forms and if needed then it can be turned on again for the cases where customer would like to have the boxes. In example: document complete etc...
