When the student fails the exam, the exam is locked after the first attempt.
However the exam object (Content->Exam object->Summary Tab->Maximum Tries) is set with value superior to 1 (ex:2).
Resolution
In the Content->Exam object, make sure that 'Lock Exam' Checkbox within Summary->Analyze the exam results is UNCHECKED. Otherwise the exam will be locked as soon as a failure actions is triggered.
