2318464 - Wrong Visit Summary is shown.

The wrong Form Template is selected when you View Summary of a Visit.

To reproduce:

1. Visits WorkCentre;
2. Visits View;
3. Open any Visit;
4. Actions > Generate Summary;
5. Wrong Summary is selected.

For Visits, only one Form is supported. So, if you have more than one, system will take the Standard.

Maintain only one Form Template for Visits.
