A Custom Calculation field is set up to display a date. An example of a date in the correct format is in the Custom Format String field. In the results, only that example date is shown in all rows, e.g.

Reproducing the Issue
Start a query in Detailed Reporting or Advanced Reporting. Add a date field to it.
Click Edit - Calculated Columns and start a new one.
Set the format to Date. Add the base date field as the source.
Click Edit Format, and in Custom Format String type any date in the format you want. Save and view the results.
The Custom Format String field receives format tokens (for date fields, examples are M, d and y) and constructs other dates according to the formatting in between. If you simply enter a date, it will contain no tokens and the exact string will be used instead of the date.
Enter a date format in token format instead, e.g. MM/dd/yyyy, save and view the results. Note that case is important, and d and y must be lower case but M means months while m means minutes.
