Resolution

You can control the display of form Templates in Admin centre by either Enable or Disable(see attached)

  1. Admin center >> Performance Management>> Form Template Settings 
  2. Or Tool Search >> Form Template Settings >> Form Template Settings

Select Enable/Disable then log out and log in
