Symptom

Simple insert on table with identity column fails with error message

  Tried to insert a duplicate value into a unique index owner.table.ASIQ_IDX_Tnnn_Innn_HG on row nnn.   -- (slib/s_ohcins.cxx   964)
----

It may occur when there is insert into same table via insert .. location which inserted explicit identity values using database option identity_insert.
