Symptom

Alert log file reports error ORA-07445: exception encountered: core dump [lnxsni()+<number>]

Following OS command returns that some SBP has already been applied for oracle database 11.2.0.4.

$ORACLE_HOME/OPatch/opatch lsinventory (UNIX)
$ORACLE_HOME\OPatch\opatch lsinventory (Windows)

However, dba_registry_history does not return the installed SBP for 11.2.0.4.

SQL> select * from dba_registry_history;
