Symptom

RLV Auto-merge and manual merge could fire:

I. 10/19 12:32:00. 0000007576    O/S Err#: 0, ErrID: 6145 (hgex_dataexception); SQLCode: -1002003, SQLState: 'QGA03', Severity: 14
I. 10/19 12:32:00. 0000007576 [20505]: Tried to insert a duplicate value into a unique index index_name on row 0.
-- (rvlib/rv_UniqueCheck.cxx 2161)

This cause RLV merge to keep on failing.

The involved table has a unique index.
