Symptom

When creating a user in SU01 or SU10 transaction code
a dump occurs with the following Runtime error: DBSQL_DUPLICATE_KEY_ERROR

Short Text    

   Open SQL array insert produces duplicate records in the database.

What happened?                                                                                    
   Error in the ABAP Application Program

   The current ABAP program "SAPLSZA0" had to be terminated because it has
   come across a statement that unfortunately cannot be executed.
