We can find this information under the "Routing Map" and "Audit Trail" sections of the Performance form:

1. Open the PM form
2. Click on Actions
3. Info about this document
4. 

To create this report in Adhoc we add the fields:

• Signer Name
• Signed Date
• Section Name

And filter the "Section Name" column by "Signatures".
