2655839 - Exam/Quiz - Failure Actions does not work, remove item from learning plan or lock out after attempts.

When using the "Upon Failure" features ("Remove Item from Learning Plan" | "Lock Item"), these are not working.

E.g.: we have created a test Assessment (Exam/Quiz) and set it to have 3 attempts, after failing 3 times the item is still in the learning plan or it will not lock out.
