2655839 - Exam/Quiz - Failure Actions does not work, remove item from learning plan or lock out after attempts.

When using the "Upon Failure" features ("Remove Item from Learning Plan" | "Lock Item"), these are not working.

E.g.: we have created a test Assessment (Exam/Quiz) and set it to have 3 attempts, after failing 3 times the item is still in the learning plan or it will not lock out.

The number of attempts set in the assessments settings and in the item settings are multiplicative, so if the assessment has an exam/quiz attempts limit of 3 and the item settings has a limit of 3, then this will be 3X3 > meaning 9 attempts before the failure action applies. Also, if the item limit is set to 3 and the assessment is set to 0 (3X0=0), which for Learning translates to unlimited.

For the failure action to apply, the "Add to History on Failure" option should also be selected in the item online content settings.

For the item to be removed from the user’s learning plan after 3 failed attempts, the following settings should be selected:

1. In the assessments settings, the "Exam/Quiz Attempts Limit" should be 1 >
   
   

2. The "Add to History on Failure" option should be selected in the item online content settings >
   
   

3. The "Upon Failure" action should be set to "Remove Item from Learning Plan" and the "Number of Failed Attempts Allowed for an Item" should be set to 3.
