Resolution

Change the following xml tags from true to false to remove the manager's firstname,last name from executive review and forms. After the change, please relaunch the forms.

includeMgrLastName="false" , includeMgrFirstName="false" for executive review

includeMgrLastNameInForm="false" , includeMgrFirstNameInForm="false" for the forms
