1.Create Item and SO
2.Add both to a catalog
3.Assign catalog to User (Assignment Profile)
4.User self-assigns Item and registers for SO
5.Remove/unregister SO
6.Prompt: Do you want to remove the associated item from the Learning Plan?
Select "Yes"
7.Item is removed from Learning Plan

If the user has enrolled into multiple offerings for that particular item in the past, then no button will not display.

Yes/No button will appear only for first schedule offering in which the user register of any item.
