Symptom
We have course home feature turned on. For some existing items there are multiple content objects associated, some content objects are contained in a folder, and other of them are not inside any folders.
Let's say we've set up a content structure like below:
As per above setting CONTENT-1, CONTENT-3, CONTENT-4, CONTENT-6 are at root level, FOLDER-5 is also at root level while FOLDER-5-1 is included in FOLDER-5 (FOLDER-5-1 is one level lower than FOLDER-5).
Assign the item to an user, with course home feature tuned on, when the user access this item and click Start Course, in user view of content structure free-floating content objects CONTENT-1, CONTENT-3, CONTENT-4, CONTENT-6 appears on the same level with the closest content object above them, whick makes them look like they are contained in the folder directory above them.

This is currently an expected behavior.
At this stage, in course home we built the UI with supporting only 2 levels on the folder structure. We do display all folders but all folders as well as content objects are flattened to be displayed in 2 levels.
Meanwhile a space for icon has been reserved in front of each content object row, consequently those free-floating content objects are pushed back a little and appear to be at the same level with other content objects inside the folder above.
Based on the new design, if all content objects have to be distinguished from each other we recommend to create a new folder for them separately.
