The FREE_TEXT values shown in the report columns are showing/working as designed.

The "Signup Form Template" column is the data for the mentoring program signup template(in other words, some of the mentoring program wizard details), and (Header ID: signupForm.sections.sectionType) field will show all the question types when admin created this program.

The "Mentee/Mentor Signup Form" section is the data for the mentee signup details, adhoc report owner will see the corresponding question type and answer for a mentee/mentor by "Mentee/Mentor Signup Form Section ID" and "Mentee/Mentor Signup Form Section Value" field.
