Symptom

SAP upgrade fails with the following error:

2EETW000 insert / update sequence failed due to an error in DBI.
2EETW000 maybe there's a concurrent process which inserted this table entry at the same time.

With the following Db2 reason code 00D70043 reported in the z/OS syslog and DB2MSTR.  The logging below references database A000XX8E, tablespace BTPXDLX.
