Symptom

While mapping database users against external identities for a SAML identity provider you get the following error:

Adding a new provider user mapping failed: There is already one valid external user name <EXTERNAL_IDENTITY> for database user <DATABASE_USER_ID> on provider <IDENTITY_PROVIDER_NAME>.
