Symptom

After transporting changes to production, or after an upgrade, the following symptoms may start to happen:

  • Email notifications are not being sent;
  • Workflow is not being triggered, the Audit Log is not showing the message "Request is pending for approval...";
  • Requests are not moving through the workflow;
  • The submit button is missing/greyed out;
