2873094 - Forms not attached to Notifications - Onboarding 1.0

PDF forms that were attached to the Onboarding notification did not send.

There is an advanced condition on the form and you forgot to check "Use Form Condition"

1. Navigate to Onboarding > Notifications
2. Find and select notification
3. Click attach > scroll right > select your form
4. Check "Use Form Condition"
   

Another reason is that the form group is selected for the notification you cannot use both functionalities together.

1. Navigate to Onboarding > Notifications
2. Find and select notification
3. Click edit and remove the form group from "Attach Form Group"
