2889595 - Fiori Logon Page does not prompt for credentials

Symptom

When you open Fiori Launchpad, the login screen cannot be displayed. It shows an empty/blank page and there are no fields to enter username or password to log on.

In HTTP trace, the following request returns 403 Forbidden:
