2907799 - Phase PARMVNT_SHD failed with SQL-error "289-cannot use duplicate index name"

Symptom

When you run SUM for upgrade, it fails at phase PARMVNT_SHD with SQL-error "289-cannot use duplicate index name"

You have followed KBA 2499221 - MAIN_SWITCH/PARMVNT_XCNV fails with SQL-error "289-cannot use duplicate index name".

However, there's no duplicate DDL (create index) commands mentioned in step 2.

Instead, the error details in SUM trace like below:
