2921339 - SCMON Extractor fails with "NONE:Open SQL array insert produces duplicate records in the database"

Symptom

The extractor "Custom Code Usage Logging - SCMON (Month DSO to Month Cube)" fails with the error log:

NONE:Open SQL array insert produces duplicate records in the database

The following ST22 dump is also encountered:
