2950980 - Performance Form Rating Labels in PDF or Print Version Show Differently

EZ rater is enabled in the form template. The rating labels showing in the form do not appear the same when printing the form or when saving the form to PDF.

Configuration of labels in Manage Languages and/or the form template XML
