Symptom

The below error is displayed when logging in via the EPM Office Add-in Excel.

Couldn't read dimension Entity members and hierarchies: Wrong csv format (invalid Wrong csv format (invalid parent): Member [Dimension_name].[PARENTHX].[MemberY] is attached to member [Dimension_name].[PARENTHX].[MemberZ] on hierarchy HX, but [Dimension_name].[PARENTHX].[MemberZ] doesn't exist.
