3021829 - Manage Substitutes tile shows error when creating substitution

After creating a custom "Manage Substitutes" tile, it displays "Error in adding substitute" when adding a substitution despite showing "Active" under the user ID. 

In the custom tile configuration, &/substitution is added as an additional parameter.
