3033888 - Checkbox on custom forms were not populated - Onboarding 1.0

• A blank form generated with mapping and data maintained
• Some mapping not work, such as checkbox or radio button
• Form is generated in New Employee Step and presented to the Manager in Orientation Step, but no checkbox option is selected on the form even though the panel has captured the value.

CAUSE 1: 

• Radio buttons are used in the mapping on the forms

CAUSE 2:

• Form template is not created through Adobe PDF Editor.

CAUSE 3: 

• The form was generated during New Employee Step and the checkbox was configured in the Orientation Step with no Manager Signature.

SOLUTION 1: Radio buttons are not supported in the forms. Rather, use checkbox form property.

SOLUTION 2:

1. Download the form template that has the issue.
2. Check Document Properties.
3. If the PDF Producer or Application is Microsoft Word or other PDF editor, please re-generate the form template through Adobe PDF Editor.
4. Upload the new form template.

SOLUTION 3:

• This is expected as the form is just previewed when there is no manager signature configured.
• With the manager signature, the form is re-generated during Orientation Step - not only preview of the form, as a result, the checkbox being kept on the form.
