3105396 - Roles requests pending in GRC automatically going to failed state in IDM

During IDM/GRC Risk Analysis you notice role requests after approval are automatically going into failed state in IDM after some amount of seconds while the requests are still pending in GRC.

You may see the following errors in the dispatcher log: 

"EXEC task: EXEC_FAILED:Missing APPROVAL_OPERATION_RESULT attribute in object:XXXXX"

"Evaluation of approvalid = <ID> failed with Exception: Object not found"

"Exception: Maximum stored procedure nesting level exceeded (limit 16). Please use sp_configure to increase the 'max nesting level'."
