3115744 - DM - cannot login to application (SAP.DM.Security.SecurityBusinessException & _RequestVerificationToken )

When trying to login to the Disclosure Management application, there is either one of these errors

Exception of type 'SAP.DM.Security.SecurityBusinessException' was thrown.

Or

http://server:port/Account/LogOn?username=username&password=pwd&returnUrl=returnUrl

Sorry, an error occurred while processing your request

In the log, you may see this error
