When 3 requests for example are sent to GRC for approval, the approver approves all 3 at the same time but only 1 request gets approved and the other 2 go into a failed status.
In the audit logs of the failed assignments you see the following error msg:
"Maximum stored procedure nesting level exceeded (limit 16). Please use sp_configure to increase the 'max nesting level'"
