3123993 - Error "No amounts maintained for the specified age group with the selected combination of fields" when enrolling Insurance Benefit

When enrolling the employee in an Insurance type Benefit, the following error is received upon save of Enrollment: 

• "No amounts maintained for the specified age group with the selected combination of fields"

1. Go to any employee > Benefits Overview
2. Try to Enroll into any Insurance Benefit
3. Fill out: Insurance Plan, Provider, Enrolling For, Coverage
   
   • Result: No error
4. Try saving the enrollment request
   
   • Result: No amounts maintained for the specified age group with the selected combination of fields

It could be either of the following:

1. Employee Contribution, Employer Contribution or Smoking fields are set as "Not Visible" in the Insurance Plan Enrollment Details Object Definition;
   
   
2. Or Employee Contribution and Employer Contribution or Smoking fields are not configured in the Benefit Enrollment Config UI;
   
   
3. Mismatch configuration of the field "Smoking" between Rate Chart and UI.

If the fields Employee and Employer Contribution and/ or Smoking are marked as Not Visible in the object definition, then the data will be saved as 'null' or 'no selection'. Consequentially, causing data issues.

Therefore, the Employee and Employer Contribution fields should not be set as "Not Visible" in the object definition. (Employee and Employer Contribution should be read-only).

When using a custom UI for an Insurance Benefit, these fields need to be defined within the Insurance Plan Enrollment UI section, and if there is any requirement to hide these from the Enrollment screen, then this should be done via the Configuration UI for Benefit Enrollment via Manage Config UI.

The visibility can be set to "No".

For the smoking field, make sure the field configuration in the Rate Chart matches what is being done during the Enrollment. 

By default, the smoking field is set to "No Selection" in the UI. So if you hide this field and your rate chart is configured to smoking as "Not Relevant", you will receive the error.
