3149678 - Error message is received by Manager while terminating candidate from EC - Onboarding 1.0

When offboarding a candidate, Manager receives notification that shows Termination failed :Failed to create offboarding activity for user : xxxxxxx due to some reason. Error Messages :[ErrorCode - 500; ErrorMessage - ]

In spite of unchecking the box 'Send email to terminated user’s manager if there is a failure in Offboarding activity creation' in "CreateOffboardingActivity" job, if the manager is still receiving the error, then please reach out to SAP Support for next steps. This will require a script execution.
