Symptom

I get the error FatalAssertionException: only one organization should exist for an OrganizationID in the following situations:

  1. Punching out to the supplier's catalog 
  2. Sending a Purchase Order (PO) to the supplier
  3. Reconciling Invoices, but the Invoice gets stuck in Approved status
