3189170 - Why learners are getting courses assigned to their to-do list on their first login?

When a learner account is created in my Litmos instance and these learners access the system for the first time, they are getting a course assigned to their to-do list.

The reports are saying the course was self-assigned but the learners never accessed the Content Library and performed the assignment action. Why and how was this assignment performed?
