Error on FAN Document, when opening PDF check boxes are misaligned from page 3 to page 2

SAP SuccessFactors Onboarding

1. Access the instance 
2. Proxy as the Hiring Manager
3. Select Onboarding
4. Search for candidate E-Verify activity 
5. Click on the PDF icon and select FAN SSA English
6. The check boxes for the section Please indicate below whether or not you intend to dispute this case in page 3 are not displaying correctly
7. If you review page 2 at the end, check boxes and signature of the candidate are overlapping on the text. 

E-Verify updated their templates and mapping is broken right now
