How to get the mailing address fields populated on CA Notice to Employee Form

The fields "Hiring Employer’s Mailing Address (if different than above)" and "Hiring Employer’s Telephone Number" are a combination of the below fields:

• CA_WTPA_CmpMailAddress1
• CA_WTPA_CmpMailAddress2
• CA_WTPA_CmpMailCity
• CA_WTPA_CmpMailState
• CA_WTPA_CmpMailZipCode

In order to have this populated on the form you can add this key on the Corporate structure, or you can add this on a Panel during the ONB Process.
