3241406 - Workflow Timer is stuck and case did not progress

We a have process using a Decision Gateway with a Timer.  However the case was stuck at the Timer and the first task was not created and assigned to the Role members.

Please check the Decision Gateway, as the Automatic Decision may not be configured correctly.

The Timer may appear to be stuck, but the problem is actually with the Decision Gateway.
For example, the following gateway is not configured correctly.

The solution is to configure the gateway with different tests for each path.
