3286390 - EPM Add-in error when logging in "Couldn't read dimension"

Symptom

The following error occurs in the EPM Add-in when trying to log in to a BPC MS connection:

An unexpected error occured while loading the dimension: <DIMENSION NAME>
Couldn't read dimension RECURSO members and hierarchies: Wrong csv format (invalid parent): member [DIMENSION NAME].[H1].[MEMBER NAME] is attached to member [DIMENSION NAME].[H1].[PARENT MEMBER NAME] on hierarchy H1, but [DIMENSION NAME].[H1].[PARENT MEMBER NAME] doesn't exist.

or

An unexpected error occured while loading the dimension: <DIMENSION NAME>
Couldn't read dimension <DIEMNSIOD NAME>members and hierarchies: Index was out of range. Must be non-negative and less than the size of the collection.
