An issue associated with a risk control could become "orphaned" if:
1) As part of an edit, change request, or review, a significant change caused the control to be removed from an engagement and
2) As a result, the control was no longer required for any engagements.
These "orphaned" issues were included in the count on the Issues tile in the dashboard, and users would receive e-mail notifications about them.
